Unique PRN opportunity to join SSM Health in our innovative direct-to-employer program in the Madison area. Our focus is on providing patient-centered primary and acute care for Sun Prairie School District employees and their families. New and recent graduates are encouraged to apply.

• Direct employment with SSM Health
• PRN position with a commitment of 2+ shifts per month preferred
• Off-site clinic locations with daytime hours (may vary slightly by location)
• No weekends, holidays, or call responsibilities
• No inbox requirements when away from the clinic
• Patient populations include primary care (age 5+) and acute care (age 2+)
• Individualized patient care approach supported by a comprehensive healthcare team including onsite labs
• Guaranteed hourly pay

• Provide independent patient care services
• Conduct comprehensive primary and acute care visits
• Perform pre-employment and work-related physicals
• Develop and participate in population health programs
• Utilize EPIC Electronic Medical Record system

• Certification as PA or FNP
• Ability to obtain Wisconsin state licensure
• Experience in primary and acute care preferred
• Strong communication skills with patients
• Commitment to delivering personalized, high-quality healthcare
