AGING CARE MANAGER 2

As an Aging Care Manager 2 with the Northumberland County Area Agency on Aging (AAA), you help older adults overcome challenges and stay safe in their communities. This position supports individuals and families by linking them to programs that make daily life easier. You make a positive impact through careful planning and thoughtful guidance. Your work strengthens the well-being of those who rely on compassionate care.

This role centers on assessing needs and coordinating services that enhance the quality of life for older adults. As an Aging Care Manager 2, you will perform the following duties:

• Needs Assessment: Evaluate social contacts, daily living activities, cognitive functioning, physical health, and available resources
• Service Coordination: Connect older adults with agencies, hospitals, clinics, courts, and community organizations that support their needs
• Comprehensive Reviews: Conduct detailed assessments of medical, social, psychological, and housing conditions to support effective care planning
• Interagency Collaboration: Work with professional staff and community partners to discuss diagnoses and plans of treatment
• Benefits Guidance: Assist older adults in managing income, accessing Social Security and Medicare, and understanding legislation that affects benefits
• Case Documentation: Prepare social studies, case histories, reports, and records

Work Schedule and Additional Information:

• Full-time employment
• Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch.
• Telework: You will not have the option to telework in this position.
• Salary: In some cases, the starting salary may be non-negotiable.

Required Experience, Training & Eligibility

Qualifications

Minimum Experience and Training Requirements:

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.

• Hiring preference for this vacancy may be given to candidates who live within Northumberland County. If no eligible candidates who live within Northumberland County apply for this position, candidates who reside in other counties may be considered.
• You must be able to perform essential job functions.
