Job Summary
:
Provide outpatient psychiatric care to client employees and their dependents 18 and older at the Off‑Campus Health Center in Sunnyvale, CA. Scheduled Monday‑Friday, 8 hours per day between 8 AM‑6:30 PM. Sign‑on bonus $7,500.

• Perform examinations and care measures for the patient population covered by the client contract and within prescribed guidelines for the provider’s scope of practice.
• Order, interpret, and evaluate diagnostic tests to identify and assess patients’ clinical problems and health‑care needs.
• Record physical findings and formulate plans and prognoses based on patient condition in the electronic medical record (EMR).
• Collaborate with and refer to medical providers, behavioral‑health professionals, and other health professionals to deliver comprehensive care plans as necessary.
• Provide medication initiation, titration, and management for members as appropriate as part of the care team.
• Refer patients to specialized health resources for treatment as indicated.
• Assist members in gaining insight, defining goals, and planning action to achieve personal, social, educational, and vocational development and adjustment.

• Medical Degree (MD or DO) with unrestricted license in the State providing care or supervision/collaborative practice agreements.
• Board certification from the American Board of Psychiatry and Neurology (ABPN) appropriate for the population served (adult, adolescent, or pediatric).
• State Prescriptive Authority (if applicable).
• Federal DEA license matching each state of practice as applicable.
• State Narcotic/Controlled Substance license if required in your state.
• CPR Certification.
• Collaborative Assessment and Management of Suicidality (CAMS) certification obtained within 12 months of hire.
• At least three years of post‑residency clinical experience as a psychiatrist in ambulatory care, occupational health, family practice, or emergency‑department settings.

Premise provides its reasonable and genuinely expected range of compensation for this job of $ – $ annually. The rate offered will depend on experience, qualifications, and location.
