, is currently seeking to hire a compassionate and experienced Full-Time RN Case Manager to join our growing home health team. This position offers the opportunity to provide personalized care in a home setting, ensuring clients receive the highest quality of service and support throughout their recovery journey.

Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinate services, counsel the client and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

• 40 hours per week 8:00 am - 4:30 pm
• Monday through Friday
• No Night Shifts (also known as PM and NOC Shifts)
• Limited on-call rotation

• $36- $40 per hour starting wage, depending on experience

• Active Wisconsin Registered Nurse License
• Two (2) years’ nursing experience, 1 year in Home Health (preferred)
• CPR Certification
• A valid Wisconsin Driver’s License and Car Insurance
