• Full‑time or part‑time Endodontist at Surprise and Avondale, Arizona
• Sign‑on bonus: $25,000

• Direct referrals from General Dentists
• Predominantly PPO practice, comprehensive dental office
• Compensation: percentage of production + $25,000 signing bonus

• Board Certified/Board Eligible
• 1 year of experience preferred
• Endodontic Residency Certificate
• Accredited Dental School Graduate
• Current applicable State Dental License (or ability to acquire by time of employment)
• Other licenses/certifications such as CPR, DEA, NPI
