*** As a
** Home Health Admit RN**, you’ll be the heart of patient care, bringing clinical expertise and compassion directly into the home. This role combines
** Start of Care (SOC), wound care**, and
** infusion therapy
** responsibilities, providing high-quality nursing care to patients in their homes. You will:
* Perform Start of Care (SOC) patient visits, including initial physical assessment, medication reconciliation, home safety evaluations
* Create personalized care plans to improve patient outcomes and support their independence
* Perform wound assessments and provide wound care treatments if necessary
* Administer and monitor IV/infusion therapies per physician orders when required
* Complete accurate and timely clinical OASIS documentation
* Act as a trusted Case Manager, coordinating care with physicians, therapists, and the interdisciplinary team
* Educate and empower patients and caregivers with confidence

*** Our Requirements
**** Currently licensed as a Registered Nurse (RN) in the State of Arizona
* Minimum of 1 year Home Health preferred
* Wound care and/or infusion experience preferred
* Strong clinical skills and a passion for patient-centered care
* Expert organizational and time management skills
* Current CPR certification
* Valid AZ driver’s license, proof of auto insurance, and reliable transportation
