Lines of AuthorityThis position is supervised by the Program Manager or their designee of the SAFE House. Unless specifically designated, this position does not supervise other personnel. The position may require periodic participation in peer review activities.
Position SummaryAdvocate for clients, obtain entitlements, coordinate transportation between SAFE House and other service providers. The BHT/Case Manager is expected to cooperate as a team member in the provision of a therapeutic and safe environment that encourages each resident’s emotional growth and security. Responsible for planning, coordination, and implementation of the residents DV education program. This position is expected to meet all standards for clinical documentation.
PrincipalDuties / Responsibilities
- Adheres to personnel policies and complies with agency requirements for ethical and professional behavior.
- Provides case management services to clients, including group facilitation for residents, children's activities, and coordination.
- Manages all aspects of clinical records, provides crisis services and assists with service coordination for the client.
- Engages and includes the client in the development and implementation of the resident's goals; reviews progress weekly.
- Protects client confidentiality and personal rights.
- Keeps clear, concise and accurate records of clinical activity according to policy and procedures.
- Assists clients in obtaining entitlements and benefits pertaining to housing, income, health, and other supportive services.
- Provides recreation, leisure, and pro‑social skills for residents and their children individually and in groups.
- Provides transportation as needed.
- Works with other agencies to provide wrap‑around services including home and/or school visits.
- Attends appropriate training and stays informed on the needs of the residents.
- Establishes and maintains working relationships with personnel, referral resources, and community partners.
- Performs vocational screenings and evaluations.
- Acts as a liaison between client, Department of Vocational Rehabilitation, and employer.
- Provides job development and placement services.
- Provides community training site development.
- Coordinates and accompanies clients to PCP/Doctor appointments.
- Provides health promotion groups including, but not limited to, hygiene, exercise, and nutrition.
- Provides housing support: identifying needs, assisting with applications, and locating resources and referrals.
- Attends scheduled meetings and trainings; participates in quality management and quality improvement activities.
- Performs other duties as assigned by supervisor or designee.
- Frequent interaction with residents, their children, and support systems.
- Coordination with peers, advocates, and team members as needed.
- Collaboration with community resources/referral agencies with the resident's consent.
- High School Diploma.
- Five (5) years of Behavioral Health Experience for BHT case management level.
- Current Arizona Driver's License.
- Compliance with a drug‑free workplace.
- At least 21 years old.
- Knowledge and ability to define the needs of clients served.
- Knowledge of symptoms related to qualifying diagnosis.
- Ability to identify and communicate signs of regression and deterioration.
- Knowledge of interventions to foster self‑care and ADL skill development.
- Knowledge and training in developmental stages and interface with childhood disorder.
- Knowledge of behavior management and ability to develop age‑appropriate interventions.
- Ability to communicate at an age‑appropriate level.
- Ability to identify signs and symptoms of substance abuse and chemical dependence.
- Knowledge of range of treatment for dependence including relapse prevention, strategies, and self‑help groups.
- Ability to provide a diagnostic impression per DSM‑IV criteria for substance abuse and chemical dependency.
