Registered Nurse; RN - Home Health - per visit

BAYADA Home Health Care is seeking an experienced full‑time Home Health Registered Nurse (RN) in Swampscott, Massachusetts. This position is ongoing and staff level.

• Use strong head‑to‑toe assessment skills to identify subtle changes in patient condition.
• Apply acute and post‑acute care experience (CHF, COPD, wound care, IV therapy, post‑surgical care).
• Triage and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Conduct comprehensive wound assessment, care, and documentation.

• Work independently while knowing when to elevate concerns.
• Make real‑time clinical decisions without immediate on‑site support.
• Strong problem‑solving skills in unpredictable home environments.
• Manage a dynamic visit schedule and prioritize urgent visits.
• Efficient with point‑of‑care documentation and technology.
• Attention to detail while maintaining productivity expectations.
• Comfortable providing care in varied home environments.
• Flexible and adaptable to changing patient needs and schedules.
• Demonstrate cultural sensitivity and respect for patient autonomy.

• Current nursing license in good standing in Massachusetts.
• Graduation from an accredited and approved nursing program (school transcript or diploma).
• At least one year of recent hands‑on medical experience with adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med‑surg, sub‑acute care or rehab units (medical or skilled nursing settings).
• Experience with OASIS documentation.

Competitive pay ranges with a weekly pay schedule. Base pay: $60–$65 per point, depending on qualifications and subject to negotiation. $5,000 bonus with a full‑time commitment.
