Alpha Care Home Health Agency is seeking a compassionate and motivated Physical Therapist (PT) to join our home health team. In this role, you will provide individualized therapy services to patients in their homes, helping them improve mobility, restore function, and maintain independence while enhancing their overall quality of life.

We are looking for a clinician who is patient-focused, organized, and confident working independently while collaborating with an interdisciplinary care team.

• Perform physical therapy evaluations and ongoing patient assessments
• Develop and implement individualized therapy treatment plans
• Provide therapeutic exercises, interventions, and mobility training based on physician orders and patient goals
• Educate patients and caregivers on exercises, safety, and disease management strategies
• Monitor patient progress and communicate changes to physicians and care team members
• Maintain accurate and timely clinical documentation in accordance with agency guidelines
• Participate in interdisciplinary care planning and discharge planning
• Assess home environments and recommend safety modifications as needed
• Supervise Physical Therapy Assistants and Home Health Aides in accordance with agency policy

• Graduate of an accredited Physical Therapy program
• Current Massachusetts Physical Therapist license
• Minimum one (1) year of healthcare experience preferred
• Home Health experience preferred
• Strong communication, organizational, and clinical skills
• Ability to work independently with minimal supervision
• Valid driver’s license and reliable transportation
