As a Home Health Care Case Manager, you will be responsible for coordinating comprehensive care for patients in their homes, with a focus on acute care and wound care management. This role involves collaborating with healthcare professionals, families, and patients to develop and implement effective care plans. Your expertise will play a crucial role in promoting healing and enhancing the quality of life for our patients.

Key Responsibilities:

• Assess patients’ needs and develop individualized care plans, particularly for acute health conditions and wound care management.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient care.
• Monitor patient progress and adjust care plans as necessary.
• Provide education and support to patients and their families regarding wound care and overall health management.
• Maintain accurate and timely documentation of patient care activities and progress.
• Ensure compliance with all regulations and quality standards in home health care.
• Participate in interdisciplinary team meetings and case reviews.

Qualifications:

• Wound care certified preferred but not required.
• Current RN or LPN license in Massachusetts.
• Previous experience in home health care or case management, with a strong focus on wound care.
• Excellent communication and interpersonal skills.
• Strong organizational skills and attention to detail.
• Ability to work independently and as part of a team.
• Proficiency in electronic medical records (EMR) systems.
