Position: Opioid Remediation Case Manager (Grant-Funded)

DeKalb County Treatment Courts are seeking a compassionate, driven Opioid Remediation Case Manager to support individuals navigating recovery from Opioid Use Disorder (OUD) and co-occurring Substance Use Disorder or Mental Health conditions. In this role, you'll be a critical link between Treatment Court participants and the resources they need to rebuild their lives - connecting clients to healthcare, housing, counseling, and community support while helping them stay on track with their treatment and recovery plans.

What You'll Do
  • Assess participant needs and build personalized care plans
  • Connect clients to housing, healthcare, financial assistance, and other community resources
  • Collaborate with healthcare providers, counselors, probation, and the Treatment Courts team
  • Monitor client progress and adherence to case plans
  • Provide crisis intervention and support during emergencies
  • Educate clients and families on available services
  • Attend court sessions, staffings, and meetings as part of the treatment team
What You Bring
  • Experience in social work, case management, or a related field preferred
  • Strong written and verbal communication skills
  • Sound judgment and problem-solving ability
  • Cultural sensitivity and the ability to connect with a diverse caseload
  • Working knowledge of community social service agencies
  • Strong organizational and time-management skills
  • Proficiency in Word and Excel
  • A valid driver's license (required for client transport)
  • Bilingual English/Spanish skills a plus
Good to Know
  • This position reports to the Treatment Courts Supervisor and Director
  • Employment is contingent upon a clear criminal background check and a TB screening test
  • Some local travel required using a Treatment Courts vehicle
  • This position isgrant-funded through 6/30/27, with renewal anticipated annually
