Position Overview
We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes across Jackson and Swain Counties. The ideal candidate will be comfortable working independently, managing a daily patient caseload, and delivering high-quality home health services.
Key Responsibilities- Provide skilled nursing care in patient homes
- Complete Start of Care (SOC), routine visits, evaluations, and other visit types as needed
- Develop and implement individualized patient care plans
- Accurately document patient assessments and clinical interventions
- Collaborate with physicians, caregivers, and interdisciplinary teams to ensure quality patient outcomes
- Active North Carolina RN License or Compact RN License
- Home Health experience
- Strong knowledge and experience with OASIS documentation
- Valid driver's license and reliable transportation
- Experience with Homecare Homebase (HCHB)
- Strong assessment, communication, and care coordination skills
