Position DescriptionThe RN Certified Diabetic Educator works collaboratively as an active member of the interprofessional care team to provide therapeutic patient and family‑centered care. The RN Certified Diabetic Educator will provide diabetes education conducive to behavior change and improved quality of life across the lifespan. The RN Certified Diabetic Educator supports and advocates for people affected by diabetes to optimize quality care.
The RN Certified Diabetic Educator promotes self‑management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes. This position reflects the mission, vision and values of the practice both internally and throughout the community.
- NYS Registered Nurse – Bachelor’s Degree preferred
- Certified Diabetes Care and Education Specialist or Board‑Certified Advanced Diabetes Management credentials. If not already a Certified Diabetic Educator, will sit for the certification exam within 24 months of hire.
- BCLS Certification
- Minimum of 3 years of direct nursing care
- Preferred:
- Diabetes self‑management education and support experience
- Working knowledge of computer systems and diabetes technology
- Highly organized and strong attention to detail
- Strong problem‑solving and critical thinking skills
- Ability to work with a diverse patient population and have exceptional interpersonal and customer service communication skills
- Experience in a physician’s office, strong clinical/clerical skills and knowledge, and good knowledge of computer systems and new technology
Pay Range: $60,000 – $74,880 per year DOE
