At Nascentia Health the Registered Dietitian will be responsible for the delivery and evaluation of medical nutrition therapy services to agency patients. This is a Per Visit role that supports patients in the Oneida County region.
Performance Responsibilities and Standards- Provide in-home consultation for policy and program development and operation and evaluation of nutrition services.
- Serve as a consultant to the agency in the application of nutrition knowledge.
- Provide direct service to patients and families through home visits. Assess nutrition status and food practices; assist in plan for the provision of food and fluids appropriate for the individual's physical and environmental conditions. Provide nutrition education and counseling to meet nutritional needs.
- Provide appropriate educational materials to staff and participate in in‑service training of staff.
- Demonstrate knowledge of and ensure compliance with agency policies and procedures. Maintain patient confidentiality.
- Demonstrate knowledge of, and ensure compliance with, federal, state, and local regulations governing home care.
- Complete nutrition care documentation in a timely manner. Communicate with staff regarding patient nutritional needs.
- Perform all other duties as assigned.
- Bachelor's degree with a major in foods and nutrition from a college or university approved by the Educational Department. Registered dietitian status with the American Dietetic Association;
New York State Certified Dietitian/Nutritionist. Maintain registration status through approved continuing education activities. - Two to three years of professional experience in a health agency including counseling patients in normal and medical nutrition therapy.
- Current New York State driver’s license and automobile insurance.
- Must be willing to drive to see patients in their homes.
- Visual and hearing ability sufficient to comprehend written or verbal communication.
- Ability to express self verbally and in writing.
- Ability to drive in varied weather conditions.
- Ability to use the telephone and communicate over the telephone.
- Bloodborne Pathogens Exposure Category: II – Normal work routine does not involve exposure, but exposure may be required as a condition of employment. Protective measures should be readily available.
Per-visit role with a minimum–maximum rate of: $25.00–$75.00, plus round-trip mileage reimbursed at the Federal IRS rate.
