Admissions Team Social Worker

Hospice of Central New York and Hospice of the Finger Lakes is a non-profit organization committed to delivering compassionate and expert end-of-life care. Serving terminally ill patients and their families across Onondaga, Oswego, Cayuga, and Madison counties, the organization focuses on comfort, dignity, and quality of life. This is a full-time, on-site position for a Social Worker. As part of the admissions team, he Social Worker will conduct a comprehensive psychosocial assessment, assessing for supportive care giving and safety. Alongside the admission nurse, the admission team provides education and instruction about Hospice philosophy and end of life care. The role requires sensitivity, empathy, and a commitment to delivering patient-centered care.

• Experience in social work, counseling, and emotional support for patients and families
• Ability to assess client needs and develop comprehensive care plans
• Knowledge of resources, community services, and healthcare systems
• Strong communication, collaboration, and interpersonal skills
• Capability to assist with grief, loss, and bereavement support
• Licensure in social work (LCSW or LMSW)
• Open to MSW with Licensure within six months of hire
• Prior experience in hospice or palliative care is an advantage
