We are currently looking for a Full Time Rehabilitation Coordinator to join our Rehab Department.
About the Role
We are seeking a detail-oriented, organized, and proactive Rehab Coordinator to support the Rehabilitation Director and therapy team in ensuring the smooth, efficient daily operation of the rehabilitation department. This role is essential in helping coordinate patient care, managing therapy schedules, supporting documentation compliance, and maintaining communication across the interdisciplinary team.

Key Responsibilities
Scheduling & Coordination
- Coordinate daily therapy schedules across all therapy disciplines.
- Review payer-specific therapy requirements to plan and coordinate service delivery for each patient.
- Review MDS schedules to identify assessment dates; update the screen-tracking tool and notify therapists accordingly.
- Assist with scheduling patients based on orders, documentation requirements, and payer guidelines.
- Complete and maintain Admission and Discharge tracker forms; ensure all therapy paperwork is complete.
- Maintain the Continuing Care Rehab tracking tool (payer source, re-certification dates, discharge dates); provide weekly updates to appropriate team members.
- Manage Medicare Part B non-coverage letters.
- Assist with auditing as requested.
- Run daily therapy order reports in PCC, review for skilled documentation, and assist therapy staff as needed.
- Monitor the Net Health dashboard to support therapy documentation compliance.
- Start new cases in Net Health, ensuring accurate insurance information and therapy minutes aligned with payer guidelines.
- Maintain required postings of therapist licensure.
- Collaborate with Human Resources to ensure all therapist licenses remain current.
- Process, distribute, and respond to incoming mail; perform filing, faxing, and copying as needed.
- Answer department phones, take messages, and relay information to appropriate staff.
- Cross-reference payroll, time and attendance, and Net Health daily to ensure accurate labor hour reporting.
- Assist with coordination and ordering of Durable Medical Equipment (DME) for patient discharges.
- Perform all other tasks as assigned by the Rehabilitation Director.
- Experience in a healthcare or rehabilitation setting preferred.
- Strong organizational and time-management skills.
- Excellent verbal and written communication abilities.
- Ability to manage multiple responsibilities in a fast-paced environment.
- Proficiency with electronic health record systems (e.g., PCC, Net Health) preferred.
- Working knowledge of payer guidelines (Medicare A & B, private insurance) is a plus.
