CNY Family Care, LLP was founded in 1999 with the goal of providing compassionate and professional medical care. We are a family‑oriented medical practice with physicians, physician assistants, nurse practitioners and diabetic educators. Our team approach meets patients’ medical needs through compassion, respect, and professional care. We are available for patients when they need us, partnering to provide a proactive approach to medical care that improves quality of life.
One of our main goals is to provide all patients with continuous and comprehensive health care. As part of this model, we ask patients to select one personal physician. The PCP/Primary Care Physician maintains an ongoing relationship with the patient and oversees the care team, consisting of nurses, nurse practitioners, physician assistants and diabetic educators. The team works to ensure patients receive appropriate age‑related preventative testing and the latest evidence‑based treatment for their conditions.
We continue to grow and expand services to meet the demands of our patient population.
