• Large, primary care office
• Full-Time
• Exempt
• $90,000 - $130,000 annually

• Annual performance review, performance‑based merit increase

We are a Primary Care practice and a Patient Centered Medical Home, who takes a team‑based approach to medicine. Our Nurse Practitioners have ready access and ability to precept cases with our Physicians in a supportive environment.

• Provide and coordinate health care services to patients and their families.
• Promote maintenance of health, prevention of illness and stability.
• Provide care during acute and chronic phases of illness.
• Guide and counsel individuals and families.
• Referral patients to other health care providers as appropriate.
• Record and update patient EMR in accordance with HIPAA and practice standards and procedures.

• Current, certified NYS Nurse Practitioner license.
• Minimum of two (2) years’ experience in a family practice or Urgent Care setting is desired (not required).
• Previous experience working with Medent (EMR) is a plus.
• Excellent working knowledge of chronic disease states and possess critical thinking skills.
• Strong interpersonal and communication skills.
