- Large, primary care office
- Full-Time
- Exempt
- $90,000 - $130,000 annually
- Annual performance review, performance‑based merit increase
- Additional supplemental compensation available
- CME allowance
- Malpractice insurance, licensing fees, registration, and DEA fees paid by the practice.

We are a Primary Care practice and a Patient Centered Medical Home, who takes a team‑based approach to medicine. Our Nurse Practitioners have ready access and ability to precept cases with our Physicians in a supportive environment.
Nurse Practitioner Responsibilities- Provide and coordinate health care services to patients and their families.
- Promote maintenance of health, prevention of illness and stability.
- Provide care during acute and chronic phases of illness.
- Guide and counsel individuals and families.
- Referral patients to other health care providers as appropriate.
- Record and update patient EMR in accordance with HIPAA and practice standards and procedures.
- Current, certified NYS Nurse Practitioner license.
- Minimum of two (2) years’ experience in a family practice or Urgent Care setting is desired (not required).
- Previous experience working with Medent (EMR) is a plus.
- Excellent working knowledge of chronic disease states and possess critical thinking skills.
- Strong interpersonal and communication skills.
