IV Infusion Nurse Home Care – Registered Nurse (RN) – Syracuse, NY.

- Possess critical‑thinking skills, decisive judgement, and the ability to work with minimal supervision in a fast‑paced environment.
- Provide in‑home infusions within the scope of practice defined by the Nurse Practice Act.
- Deliver quality care to patients requiring intravenous therapy in the home or an alternate setting.
- Administer and teach self‑administration of specialty pharmacy infusion therapies to patients with rare, chronic, and acute diseases in the home setting, infusion suites, or physician office.
- Expertly access and maintain SPC/PIV, CVAD (PICC/midlines, implanted ports), SCIG, and phlebotomy; follow proper infection‑control measures.
- Infuse IVIG, SCIG, Alpha‑1 anti‑trypsin products, biologic response modifiers, and enzyme replacement products in the home setting; administer and train patients on injectables.
- Coordinate the planning for delivery of nursing care with the Director of Nursing and administer appropriate clinical decisions in patient care delivery.
- Proactively update client/patient POC, health changes, and related incidents.
- Demonstrate knowledge of age‑specific differences in the patient population, including physiological and developmental differences unique to each group.
- Ensure safe care to patients; adhere to all policies, procedures, and standards, including time management, supply management, productivity, and quality of service.
- Travel within the assigned geographic region as required; remain flexible to work alternate shifts on short notice and be on call for field visits as determined by business needs.
- Administer CPR and other emergency procedures, as necessary.
- Licensed as a Registered Nurse through the NY State Board of Nursing.
- Home health experience preferred.
- Minimum of two years medical‑surgical or critical care/ER experience.
- Minimum of one‑year recent infusion experience.
- Proof of current PPD and current CPR – hands‑on class required.
- One year experience in infusion nursing and home health experience preferred.
