Schedule: Full-Time

Salary per annum: $150,000 - $200,000

We are seeking a friendly and compassionate Endocrinology Nurse Practitioner/Physician Assistant to join our team. As an Endocrinology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with endocrine disorders, including diabetes, thyroid conditions, and other hormone‑related issues. This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals.

• Assist care coordinators across the different providers and settings.
• Promote better health outcomes through communication between facilities and patients.
• Assess patients with endocrine conditions and report through our proprietary software system.
• Cross‑train in other areas of care management.
• Work under the supervision of an off‑site collaborative physician.
• Gather and analyze patient information.
• Perform detailed review of the system.
• Review all pertinent patient labs, studies, and provider notes.

• Minimum 6 months of experience in long‑term care or a skilled nursing facility.
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• DEA license.
• CPR BCLS/BLS certified.
• Master's degree in Nursing.
• Completed a degree from an accredited Nurse Practitioner program.
• Year of experience in Endocrinology.
• Preferred experience documenting EMR (Sigma, Visual, PCC, etc.). Training provided.
• Great organizational and time‑management skills.
• Reliable transportation.
• Willingness to travel to different nursing homes within the state of Florida (100% commitment).
