Provide high‑quality nursing care to adult patients through a combination of hands‑on clinical care, assessment, education, and care coordination. You’ll be trusted, valued, and empowered to use your clinical judgment to make a meaningful difference.

• Deliver safe, patient‑centered nursing care in the home.
• Serve as the primary case manager, coordinating care with the interdisciplinary team.
• Develop and implement individualized plans of care.
• Provide patient and caregiver education, support, and disease management teaching.
• Ensure accurate, timely documentation (documentation time built into the home visit).
• Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient’s plan of care.
• Maintain compliance with all infection control and regulatory standards.
• Stay current with clinical best practices.

• Current NYS Registered Nurse license (from an accredited program).
• BSN preferred (tuition reimbursement available).
• Valid driver’s license, auto insurance, and reliable vehicle.
• Self‑directed, organized, and comfortable working independently.
• Adult medical‑surgical, home care, case management, or chronic disease management experience a plus.
• Strong communication and computer skills.

• Ability to sit or stand for long periods during the work day.
• Ability to perform tasks involving physical activity (including but not limited to: pushing/pulling on average 50 lbs with a max of 150 lbs; lifting/carrying on average 25 lbs with a max of 50 lbs; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs).
• Ability to drive in all weather conditions (rain, sleet, snow, wind).
• Visual/hearing ability sufficient to comprehend written or verbal communication.
• Ability to use telephone and communicate over the phone.
• Frequently uses a laptop computer to enter orders and obtain product information.
• Bloodborne Pathogens Exposure Category I – Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.

Min‑max rate: $75,250 – $87,138.92 per year. Includes:

• $15,000 sign‑on bonus.
