Licensed Practical Nurse

Bishop Rehabilitation & Nursing Center is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. As a Licensed Practical Nurse at Bishop Rehabilitation & Nursing Center, you will provide direct, hands‑on nursing care to a diverse population of elderly residents requiring long‑term care, post‑acute rehabilitation, and recovery support. You will be responsible for performing thorough patient assessments within your scope of practice, monitoring vital signs, documenting clinical findings, and promptly reporting significant changes in condition to the supervising Registered Nurse and medical team.

You will administer medications and treatments safely and accurately, including oral, topical, and injectable medications in accordance with physician orders and facility policies, and you will assist with wound care, dressing changes, and other skilled nursing procedures as appropriate.

In this role, you will play a key part in implementing individualized care plans, ensuring that each resident’s clinical needs, comfort, and personal preferences are supported. You will collaborate closely with RNs, physicians, therapists, social workers, dietitians, and certified nursing assistants to coordinate and deliver comprehensive, patient‑centered care. Your daily responsibilities will include maintaining accurate and timely documentation in the electronic medical record system, adhering to all infection control and safety protocols, and supporting fall prevention, skin integrity, hydration, and nutritional initiatives.

You will also provide ongoing education and emotional support to residents and their families, helping them understand diagnoses, medications, and care routines. As a visible and accessible member of the care team, you will help create a respectful, dignified, and home‑like environment that reflects our commitment to ethical practice and family‑focused care. You will actively participate in shift handoffs, care conferences, and quality improvement efforts, offering observations and suggestions that contribute to better clinical outcomes and resident satisfaction.

The Licensed Practical Nurse will be expected to effectively prioritize and manage multiple clinical tasks in a fast‑paced skilled nursing and rehabilitation setting, respond calmly and competently to changes in resident status, and follow all state and federal regulations governing long‑term care and rehabilitation facilities. You will uphold professional standards of practice, maintain resident confidentiality, and support a culture of teamwork, accountability, and continuous learning.
