Position SummaryThe Resident Care Manager is responsible for coordinating and overseeing the daily delivery of resident care services at Francis House. This role ensures that residents receive compassionate, person-centered support consistent with the organization’s mission and standards of care. The Resident Care Manager works closely with caregivers, volunteers, families, hospice partners, and the interdisciplinary team to promote comfort, dignity, and quality of life for each resident.
EssentialJob Duties and Responsibilities (Additional duties may be assigned) Resident Care Coordination
- Oversee day-to-day-to-day resident care operations and ensure continuity of care across shifts.
- Coordinate admissions, resident transitions, and care planning in collaboration with hospice and healthcare partners.
- Monitor resident needs and advocate for timely communication of changes in condition to the appropriate clinical staff.
- Maintain accurate and confidential resident documentation in compliance with organizational and regulatory requirements.
- Prepare resident medications for administration by un-licensed staff as outlined by NYS guidelines.
- Will share responsibility as back-up to the On-Call Administrators.
- Provide supervision, guidance, and support to caregivers and resident care staff.
- Assist with staffing coordination, scheduling, and shift coverage as needed.
- Participate in onboarding, orientation, training, evaluating and ongoing development of staff and volunteers related to resident care practices.
- Foster a collaborative, respectful, and mission-driven team environment.
- Off-shift volunteer support.
- Promote a welcoming, compassionate environment that honors resident preferences, culture, and dignity.
- Support families through communication, education, and coordination with community and hospice resources.
- Serve as a point of contact for residents and families regarding care coordination and support services.
- Evaluate effectiveness of plan of care and advocate for changes as needed.
- Provide follow-up phone calls to potential residents.
- Ensure adherence to Francis House policies, safety procedures, and applicable regulations.
- Participate in quality improvement initiatives, incident review, and problem-solving related to resident care.
- Monitor supplies and equipment needed for resident care and coordinate replenishment as appropriate.
- Support and preserve the home-like, peaceful, and sacred environment of Francis House
- Foster a culture of belonging, emotional safety, and relational care
- Collaborate respectfully with residents, families, volunteers, and coworkers
- Participate in mission integration and formation opportunities

- Mentor caregivers in sacred caregiving.

This position supervises the Scheduling and Orientation Associate, Caregivers, and On-Call Administrators.
