Geriatric Nurse Practitioner
Location: City of Syracuse
$40,000 Student Loan Repayment OR $30,000 Sign‑On Bonus
In Home Care, No Nights, Weekends or Holidays
This position requires daily travel to patient homes within the assigned area.
Primary Responsibilities- Conduct comprehensive in‑home assessments, including past medical history review, medication reconciliation, vital signs, and physical exam.
- Perform evidence‑based screenings and point‑of‑care testing as appropriate.
- Manage members through acute or exacerbation of chronic conditions, including testing, diagnostics, and interventions.
- Manage members during short‑term sub‑acute rehabilitation episodes.
- Ensure accurate and complete documentation, including ICD‑10 coding.
- Communicate and collaborate with the interdisciplinary care team.
- Conduct advanced illness and advanced care planning conversations.
- Provide patients and caregivers with counseling and education.
- NP:
Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care. - PA:
Current NCCPA certification and state licensure. - Active, unrestricted licensure in the state of practice (or ability to obtain by start date) without disciplinary actions.
- Active and unrestricted driver’s license and reliable transportation for travel.
- Current active DEA licensure/prescriptive authority or ability to obtain post‑hire.
- Ability to gain a collaborative practice agreement, if applicable in your state.
- Ability to navigate varied environments and ability to sit, stand, kneel as needed to perform duties.
- 1+ year clinical experience as an APC (family, geriatric, or home health preferred).
- Experience in geriatric medicine, long‑term care, senior living, or home care settings.
- Experience meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
- Understanding of geriatrics and chronic illness.
- Understanding of advanced illness and end‑of‑life discussions.
- Proficiency with electronic medical records and technology.
Compensation ranges from $125,000 to $175,000 total cash, including base pay and bonuses. We offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase plan, 401(k) contribution, and other benefits subject to eligibility.
