Location: City of Syracuse
United Health Group is seeking a Nurse Practitioner or Physician Associate for in-home care delivery. You will travel daily to patient homes within the assigned area, performing comprehensive in-home assessments, managing acute/chronic conditions, and collaborating with an interdisciplinary care team to coordinate care.
Strong documentation and patient education are essential. Active licensure, NCCPA or ANCC/AANP certification, DEA authority, and a valid driver’s license are required.
