Full Time - Nurse Practitioner or Physician Assistant

Seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) for in-home assessments to improve patient health outcomes.

Responsibilities Include

• Conducting comprehensive in-home health assessments, including medical history, physical exams, screenings, and point-of-care testing.
• Identifying diagnoses, documenting findings, and collaborating with primary care providers to address care gaps.
• Recognizing urgent situations and providing patient education on disease management and social determinants of health.
• Referring patients to appropriate resources and ensuring follow-up care.

• Active, unrestricted licensure in the practicing state and relevant certification (ANCC, AANP, or NCCPA).
• Valid driver’s license and reliable transportation for home visits.
• Ability to navigate diverse home environments and perform physical assessments.
• Preferred: 1+ years of clinical experience, proficiency with EMRs, and strong communication skills with geriatric or Medicare populations.
