Licensed Practical Nurse

Bishop Rehabilitation & Nursing Center is seeking a Licensed Practical Nurse to join our Healthcare & Medical Services team. In this skilled nursing facility, you will provide direct patient care, administer medications, monitor resident conditions, and support rehabilitation and recovery for elderly residents. Responsibilities

• Provide direct nursing care to elderly residents in a skilled nursing and rehabilitation setting within LPN scope of practice.
• Perform routine assessments and monitor residents' conditions, including vital signs, pain levels, and responses to treatment, and report significant changes to supervising RN or provider.
• Administer medications and treatments safely and accurately in accordance with physician orders and facility policies, including oral, topical, and injectable medications as permitted by state regulations.
• Assist with wound care, dressing changes, catheter care, blood glucose monitoring, and other skilled nursing procedures as appropriate.
• Implement and support individualized care plans, collaborating with RNs and the interdisciplinary team to promote optimal clinical outcomes and resident comfort.
• Document all care, assessments, medications, and interventions accurately and in a timely manner in the electronic medical record system.
• Uphold infection control, safety, and fall-prevention protocols, and support initiatives to maintain skin integrity, hydration, and nutrition.
• Educate and support residents and their families regarding diagnoses, medications, treatments, and self-care practices, promoting understanding and participation in the care plan.
• Participate in shift reports, care conferences, and quality improvement activities, sharing observations and contributing to continuous improvement of care processes.
