Bishop Rehabilitation & Nursing Center is seeking a Licensed Practical Nurse to join our Healthcare & Medical Services team. In this skilled nursing facility, you will provide direct patient care, administer medications, monitor resident conditions, and support rehabilitation and recovery for elderly residents. You'll collaborate with an interdisciplinary team to ensure high-quality outcomes in a compassionate, family-focused environment. Responsibilities
- Provide direct nursing care to elderly residents in a skilled nursing and rehabilitation setting within LPN scope of practice.
- Perform routine assessments and monitor residents' conditions, including vital signs, pain levels, and responses to treatment, and report significant changes to supervising RN or provider.
- Administer medications and treatments safely and accurately in accordance with physician orders and facility policies, including oral, topical, and injectable medications as permitted by state regulations.
- Assist with wound care, dressing changes, catheter care, blood glucose monitoring, and other skilled nursing procedures as appropriate.
- Implement and support individualized care plans, collaborating with RNs and the interdisciplinary team to promote optimal clinical outcomes and resident comfort.
- Document all care, assessments, medications, and interventions accurately and in a timely manner in the electronic medical record system.
- Uphold infection control, safety, and fall-prevention protocols, and support initiatives to maintain skin integrity, hydration, and nutrition.
- Educate and support residents and their families regarding diagnoses, medications, treatments, and self-care practices, promoting understanding and participation in the care plan.
- Participate in shift reports, care conferences, and quality improvement activities, sharing observations and contributing to continuous improvement of care processes.
