Home Health Licensed Practical Nurse

• Provide personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN
• Implement individualized care plans with patients, families, and the healthcare team
• Educate patients and families on disease management, treatment options, and self-care techniques
• Maintain accurate patient-care records, including visits, interventions, and outcomes
• Communicate promptly with patients, physicians, clinical managers, and healthcare team members about needs and status changes
• Participate in care coordination and discharge planning
• Help patients regain independence while recovering from chronic conditions, injuries, illnesses, surgeries, or hospitalizations

• Current nursing license in the practicing state
• Valid driver's license
• Auto insurance
• Reliable transportation
• Current CPR certification
• Two years of experience as an LPN/LVN in a clinical setting
• Nursing experience in a Home Health or Hospice setting (preferred)
• Must maintain personal vehicle liability insurance in accordance with residing state minimum required limits or specified higher limits
• Required to undergo TB screening if selected

Demonstrates expertise in providing personalized nursing care, implementing individualized care plans, and educating patients and families on disease management. Proficient in maintaining accurate patient records and effective communication with healthcare teams to ensure optimal patient outcomes.
