- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments
- Develop and implement individualized care plans with physicians and healthcare professionals
- Monitor patients' conditions and report changes
- Educate patients and families on disease management, medication, and treatment options
- Maintain accurate patient care records
- Coordinate with healthcare professionals
- Report patient care and condition progress to the patient's physician and Clinical Manager
- Oversee Home Health Aides and monitor completion of documentation
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license
- Auto insurance
- Reliable transportation
- Must maintain personal vehicle liability insurance
- One year nursing experience (preferred)
- Home Health experience (preferred)
- Must complete TB screening if selected
Demonstrates expertise in providing skilled nursing care, including medication administration, wound care, and patient assessments, while effectively collaborating with healthcare professionals to develop individualized care plans. Maintains accurate patient records and oversees Home Health Aides to ensure quality care delivery.
