Job Summary

the SUNY Upstate Waters Center for Children’s Cancer and Blood Disorders is seeking an Advanced Practice Provider (APP) to deliver high-quality care to children with Hematologic Disorders and Oncologic Diagnoses
. This role is integral to enhancing continuity, quality, and coordination of care for a complex Pediatric patient population
. The primary focus of this role is on Hematologic Disorders
.

Position is based at a Level I Trauma Center that provides Critical Care and services to patients across a 17+ County Region. Scheduling is determined based on operational needs.
Upstate operates 24 hours a day, seven days a week
, providing continuous care to our region.

The APP Will Provide Expert Clinical Care Including
  • Comprehensive health assessments, including initial referrals and follow-up visits
  • Prescribing treatments for hematologic disorders
  • Ordering interventions based on clinical assessments
  • Working collaboratively with the multidisciplinary team
  • Building and maintaining strong relationships with patients, families, and referring physicians
  • Participating in quality improvement projects
Minimum Qualifications
  • Master’s Degree as a Nurse Practitioner or Physician Assistant (or graduated from an accredited PA program) or eligibility by time of appointment.
  • NYS licensed/registered NP or PA.
  • Current national certification consistent to the clinical practice.
  • Have an NPI number and are Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligibility for Medicare enrollment within six months of hire).
  • CPR‑BLS certification required at the time of appointment.
  • Must successfully pass pre‑employment and credentialing clearances.
Preferred Qualifications
  • Pediatric Hematology‑Oncology experience.
  • Ambulatory experience.
Work Days

Monday – Friday, days with holiday rotation. Scheduling based on operational needs.
