Location: City of Syracuse
AccessCNY, Inc. in Syracuse, NY is seeking a Nurse Case Manager for the Enhanced Step-Down Program.
Location:
1603 Court St., Syracuse, NY 13208. Pay rate: $37.45-$40.01 hourly.
Schedule:
Mon-Fri 8:00 a.m.–4:00 p.m. Sign-on bonus of $3,000.
The Nurse Case Manager will monitor participants’ medical care and ensure health needs are met while enrolled in the ESD Program for CTI-only services and/or the Transitional Residential Setting (TRS).
