Scope And Purpose Of Position:
My name is Gena, and I am the Manager for the Preventive Service Program (PPS) Team. The program is seeking a new member to specialize in Case Management services. As a member of the PPS Team you will work directly with children and caregivers in their homes providing education, support, and guidance that promotes safety, stability, and healthy development. We assign Case Planners to a case load of 14–20 families in Onondaga County.
The goal is to reduce risks for the families and prevent foster care placements. This role offers a dynamic blend of relationship‑building, problem solving, and community engagement.

Starting rate $23.00 per hour with a 40‑hour work week. Position is non‑exempt hourly, overtime eligible.
Responsibilities- Provide comprehensive intake and assessments of assigned clients.
- Engage and maintain regular contact with assigned clients, meeting regulatory requirements with regular home visits of significant duration, minimally twice monthly (one contact per month with children present).
- Maintain comprehensive assessments, devising and implementing individualized service plans in collaboration with the Department of Children and Family Services (DCFS) and other service providers, utilizing strategies of intervention with consideration to family strengths and cultural background.
- Provide casework/counseling, crisis intervention, brokerage and networking advocacy, coordinate services and make referrals for auxiliary services under direction of supervisor and DCFS Case Manager.
- Timely completion of required Connections documents, assessments, case narratives, case plans, statistical information, reports and outcome data.
- Maintain liaison activities with collaborative partners.
- Provide case recommendations to the Onondaga County DCFS regarding family engagement, clinical needs or other developmental needs for youth, and to other service providers regarding client utilization and needs for services, mental health/substance abuse, youth or family safety.
- Demonstrate cultural competence and integrate understanding into daily responsibilities, meeting objectives of The Salvation Army, Syracuse Area Services Strategic Plan.

Qualifications- Minimum education:
Bachelor’s degree from a four‑year college or certificate. - Required experience:
One year of related work and/or internship experience (e.g., Social Worker, Case Worker, Program Manager, Coordinator). - Must have access to private transportation and a valid NYS Driver License.
