We are seeking a talented Community Case Monitor I to join our team of nearly 500 employees. CCSI is contracted by Onondaga County to provide program management services, including specialized staffing. The Community Case Monitor I will be located in Onondaga County and have a primary responsibility of monitoring all aspects of Enhanced Voluntary Agreements (EVA) as well as AOT Court Orders as necessary.

The Community Case Monitor I will support the entire AOT in engaging individuals, coordinating team responses and ensuring the needs of the program are addressed. This role is responsible for program tracking of AOTs and EVAs, managing records, program level data collection, auditing, reporting and supporting the responses for AOT/EVA requests. The position will also provide support and oversight for a small caseload of EVA and AOTs for high-needs individuals with serious mental illness who have a history of medication non-compliance and who live in Onondaga County.

The Community Case Monitor I will coordinate and facilitate multi-disciplinary teams that include: the client, behavioral health treatment providers, case management, and housing supports, as outlined in the treatment plan. Supervision will be provided by the Forensic Case Monitor Supervisor, in close collaboration with the AOT Coordinator and Director of Forensic and Outreach  this role, you will Provide administrative support to the AOT/EVA programs.

Support and track clinical record requests.

Assist in maintaining digital and physical files.

Aid AOT/EVA programs in assessing and acquiring needs for success of program.

Assess needs of assigned and referred individuals.

Coordinate the development of system-wide treatment and support plans to include treatment services, case management, housing, transitional support services and other identified supports.

Monitor client’s participation in and compliance with an established treatment plan.

Schedule, plan, and facilitate team meetings.

Communicate effectively with the client, providers, families, and criminal justice personnel to assist with developing and sustaining the plan.

Create a sustainable treatment team by assigning roles and tasks to the treatment team to prepare for the transition to a lower level of care.

Monitor treatment implementation and coordinates action when treatment, service, and support plan needs adjustment.

Recommend adjustments to treatment plans and advocate for system movement.

Track individuals across multiple systems of care over time to assure continuity of linkages and support.

Work closely with treatment providers, ACT, FACT, case management, housing providers, parole, probation, Peer Bridger, and unique services and supports.

Culturally responsive to the population being served in the development, design, monitoring, evaluation, and/or implementation of programs and services.

Organize interviews and/or meetings with referred individuals and providers involved in the plan as needed.

Record progress notes in a comprehensive, timely manner.

Cultivate partnerships with community providers to best support clients.

Supports community education requests.

Request and review clinical records as needed.

Initiate, assist in, and complete investigations for appropriateness of AOT Court Orders and EVAs.Attend staff and other community meetings.

Maintain and organize electronic and paper records, update tracking sheets, and maintain files for the program, including archiving records.

Identify creative solutions to complex problems utilizing available community resources.

Support clients in achieving their program goals.

Develop, track, and report program goals and objectives.

File maintenance for the program, including filing and archiving of records.
