You will conduct psychosocial assessments, support admissions and ongoing eligibility, and advocate for participant rights and preferences.
You will coordinate with families, nursing home staff, and clinicians, contributing to care planning, crisis intervention, and documentation within the EMR while upholding HIPAA and regulatory
