A Medical Care Manager contributes to AHF by utilizing their experience, skills, and education together with a thorough understanding of the Ryan White Care Act Program within their specific area. With a caring and knowledgeable focus, you will work with our providers and healthcare center team so that every Ryan White client receives the support they require and access to available resources.
This includes providing education and assistance so that eligibility for the program is understood.
Working directly within the Healthcare Center, you will meet with clients and complete Needs Assessments, develop Action/Care Plans, and provide ongoing education to patients so that they understand their diagnoses and their medications. For Ryan White contracts that combine psychosocial needs within Medical Care Management, you will understand mental health needs and basic needs such as housing, food, and clothing and develop relationships with programs and agencies serving those needs and facilitate referrals, support, and assistance for patients who lack those services.
You will enhance the reputation of AHF by utilizing your case management experience to meet clients where they are, identify strengths and opportunities for growth, and assist with realistic goal setting. With collaboration and engagement, you will bring the essence of the Ryan White Care Act — the safety net for uninsured HIV/AIDS patients — to our most vulnerable population. By establishing Needs Assessments, Action/Care Plans, referrals, and support, your work will contribute to AHF’s success in meeting the deliverables of our Ryan White contract.
Beyond the contract, your work contributes to HAB Measures and DoHHS objectives.
You will be a passionate advocate for our top initiatives. To be successful as a Medical Care Manager it is crucial to ensure that the AHF Core Values and Mission stay top of mind in all you do. The Medical Care Manager will work as part of a close-knit team to be the liaison between provider and client and can facilitate the shift from leaving the exam room to implementing the optimum plan of care. You will provide brief interventions focused on long-term achievable goals, working with the client and the MCM toward viral suppression, better health outcomes and quality of life.
and Certifications
BS/BA in Social Work, or another social service field with two (2) years of direct service delivery.
Bilingual in English and Spanish preferred.
Familiarity with housing case management, and housing entitlement systems and procedures.
HOPWA Training Modules.
