Transitional Care Coordinator, MA-C

Pacific Medical Specialty Group, LLC is seeking a Certified Medical Assistant to serve as a Transitional Care Coordinator supporting patients after hospital or skilled nursing facility discharge.

In this field-based role, you’ll help ensure safe, smooth transitions back to home by coordinating care, educating patients, and closing gaps that can lead to readmissions.

• Post-discharge patient outreach and follow-up
• Medication reconciliation and care coordination
• Obtain vitals and clinical updates (within MA scope)
• Educate patients and caregivers on care plans
• Collaborate with providers, facilities, and home health agencies
  
  Timely EMR documentation
• Travel to facilities and patient homes across Washington State to assist with Telehealth Appts

• Completion of accredited MA program
• Current certification required

• Strong communication and organizational skills
• Comfortable working independently in the field

• Post-acute or care coordination experience preferred
