VAPSHCS is hiring a Registered Nurse Care Manager (RNCM) to assist with virtual Patient Aligned Care Team (PACT) coverage across VA Puget Sound Primary Care. The PACT RN Care Manager will be responsible for providing temporary and long-term virtual coverage as needed for PACT Teams across various Primary Care sites to include but not limited to:
Seattle, Mt. Vernon, American Lake, Port Angeles, Silverdale, Women's Health Clinic, Puyallup, Olympia, Everette, Oak Harbor, Auburn, and other clinics. The RNCM is responsible for organizing and directing team members to achieve desirable patient outcomes through day-to-day and/ or telephonic case management. The RNCM uses the nursing process and evidence-based practice to collaborate with the Core Team in developing the patient-driven care plan.

The RNCM will provide face-to-face clinic care to support the clinic setting.

Duties include but are not limited to:

* Providing temporary virtual coverage for multiple PACT Teams at a time across various sites- coverage to include, but not limited to, telephone triage, secure messaging, and responding to electronic chart messages "view alerts".

* Collaborating with onsite PACT teams and/or virtual care providers as needed to resolve patient care issues.

* Demonstrating clinical knowledge in assessing, planning, implementing documenting and evaluating care for a designated group of patients across the continuum of care.

* Triaging and applying a collaborative team approach in identifying, analyzing and resolving patient care problems.

* Collaborating with patients to assess and identify needs, issues, care goals and resources in critical factors for achieving desired outcomes for discharge, post hospital recovery and health maintenance/wellness.

* Maintaining current knowledge of multidisciplinary resources, programs and services, referring patients for community resources as appropriate.

* Utilizing problem-solving methodologies and advances clinical knowledge to resolve system and/or processes that impact patient care.

* Assisting with navigation of health systems and intervening or referring as needed to facilitate the achievement of patient outcomes.

* Providing care, respecting patient's personal values, cultural and belief systems.

* Assisting with the development, assessment, and monitoring/ evaluation of comprehensive plans of care to ensure efficient, effective, and coordinated care in meeting and promoting standards of wellness.

* Coordinating an interdisciplinary approach to assure physical, psycho-social, and environmental needs and resource delivery to achieve patient goals.

* Performing other duties as assigned.

Work Schedule:

Monday through Friday, 7:30AM to 4:00PM.
