A few of the Essential Duties of this role:
* Ensures agency compliance with all state/federal hospice laws & regulations
* Reviews all incoming referrals for appropriateness & manages approvals & assignments
* Reviews patient charts for medical diagnoses, procedures, medications, & clinical course
* Assists the DCS with planning, implementing & evaluating in-services & CEU programs
* Assists with interviewing, hiring, training, & mentoring organization personnel
* Participates in QAPI, OASIS review, and coding workflow
* Provide symptom management and comfort care for hospice patients in their home
* Must be willing to take call when needed
* ** A minimum of 1 year of health care management experience, preferably hospice**
* ** Must have at least 1 year of hospice agency experience, including admissions
*** Must be experienced in HOPE charting requirements for hospice
* A BSN degree is preferred, but not required
* Current BLS certification
* Current Washington state Registered Nurse (RN) license
* Experience with Home Care Home Base preferred, but not required
* Knowledge of hospice regulations, including Medicare COPs, PDGM, VBP
* Licensed and insured driver with reliable transportation
