Impact Health PC is growing and we are a post-acute practitioner group servicing skilled nursing facilities on-site and patients virtually  Nurse Practitioners and Physician Assistants are expected to provide the highest level of clinical support, consultation and treatment in collaboration with physicians, nurses and specialists. Our role is vital in achieving progressive patient outcomes and moving them toward better health.

Our mission is to provide a "whole" treatment plan that is patient‑centered with a comprehensive team approach designed to make a difference in the lives of our most vulnerable population.

If you have experience in skilled nursing, hospital emergency department, SICU/ICU, long‑term rehab or similar experience diagnosing, treating and prescribing, this role could be the perfect fit. You will be a vital member of the medical staff team and can take the lead in collaborating with nurse management and administration to achieve optimal outcomes and treat in place.
