Puget Sound Home Health and Hospice is seeking an experienced and caring full-time Hospice Registered Nurse Case Manager (RN) in Pierce County, Washington!

Complete comprehensive in-home patient assessments
Manage patient comfort, symptom control, and pain relief
Develop and update individualized plans of care under physician orders
Deliver skilled, compassionate nursing care in the home setting
Document care accurately and promptly per clinical and legal standards
Participate in quality improvement initiatives (QAPI)
Collaborate with an interdisciplinary team to ensure holistic patient support
What We’re Looking For:
Current RN license in WA (or compact state)
BLS certification from AHAValid driver’s license, auto insurance, and reliable transportation
Strong critical thinking and independent problem-solving skills
Hospice agency experience preferred but not required
