Practical Nurse; Outpatient

Duties

Provide practical nursing care to patients in the outpatient clinic with a variety of medical conditions, performing tasks such as ordering labs, radiology orders and referral results, and making future appointments.

Initiate and maintains medical histories on patients, records observations, and identifies symptoms for use by the clinician.

Conduct focused nursing assessments and contributes to the care plan in collaboration with the healthcare team.

Educate patients and families about prescribed treatments, reinforcing adherence to the medical plan.

Coordinate care across specialty care settings and in the community.

Document care in the electronic health record.

Collaborate with the healthcare team to develop and adjust care plans based on patient progress and feedback.
