Registered Nurse - Home Health

Home Health Registered Nurse (RN) Pierce County | $45–$60/hour | Full-Time Join Puget Sound Home Health & Hospice, voted one of the Best Companies to Work For in the West! • Assess patients in their homes and identify their individual care needs
• Develop and manage personalized plans of care in collaboration with providers
• Provide skilled nursing care, education, and support to patients and families
• Coordinate care with therapists, physicians, and other members of the interdisciplinary team
• Monitor patient progress and adjust care as needs change
• Complete accurate, timely clinical documentation
• Participate in quality initiatives that help us continually improve patient care

• Current Registered Nurse (RN) license
• Current AHA BLS certification
• Home Health and/or Hospice experience preferred
• Valid driver’s license, insured vehicle, and reliable transportation
• Ability to work independently while collaborating with a clinical team
