About This Role
The Hospice Visiting Nurse
- Admits/Interventionist provides comprehensive skilled nursing care to patients in their homes, focusing on admissions and follow-up visits. This role is ideal for nurses with prior home health or hospice experience who are looking to work in a supportive interdisciplinary team.
- Pay: $49 - $82/hour
- Location: Tacoma, WA
- Schedule: Full Time, 2nd Shift (Mon
- Fri 2:00 PM - 10:30 PM, Rotating Weekends 12:00 PM - 8:30 PM)
- Communicate and collaborate with interdisciplinary team members and patient families in planning and implementing care.
- Record accurate and pertinent data in the electronic medical record in a timely manner.
- Provide nursing interventions requiring specialized nursing skill and initiate appropriate preventive and rehabilitative nursing procedures.
- Follow through the plan of care to provide instruction to patients/caregivers about health care and disease management.
- Direct and supervise the care and services of LPN and HHA in the home setting.
- Perform skilled procedures including injections, dressings, catheter insertion and care, and blood draws.
- Assess patient need for care and communicate with other health care providers appropriately.
- Develop and initiate the plan of care based on assessment and collaborate with medical doctors.
- Evaluate patient response to care through timely assessment and modify the plan of care as needed.
- Prior home health or hospice experience required
- Experience in skilled nursing care in home health, palliative care, and hospice programs
- Current RN License in Washington
- BLS certification required
