Position SummaryThe Outreach and Engagement Specialist works with a multidisciplinary Homeless Outreach Stabilization Team (HOST) to provide services to homeless adults experiencing severe mental illness and substance use disorders. The role focuses on outreach, engagement, coordination of care, and documentation in community settings.
Essential Duties and Responsibilities- Provide outreach and engagement services to homeless individuals with co‑occurring disorders using the Relational and Engagement Model (ROEM).
- Consult and train homeless providers regarding target populations.
- Coordinate and link individuals to community resources and treatment agencies.
- Assist clients in finding stabilization and transition services, including psychiatric, medical, and criminal‑justice treatment.
- Document work in required systems, including FM, CMIS, HRIS, and other databases.
- Participate in community meetings, peer review, supervision, intake, diagnostic staffing, and in‑service trainings.
- Maintain charting, correspondence, and administrative paperwork timely and accurately.
- Engage with clients in emergency shelters, transitional shelters, encampments, transportation hubs and other community locations.
- Travel to client sites as required; use FBH owned or personal vehicles.
- Perform other duties as assigned.
Required
- Bachelor’s degree in a behavioral health related field.
- Must apply for Agency‑Affiliated Counselor Registration within 30 days of hire.
- Unrestricted and valid driver’s license, active car insurance and willingness to use personal vehicle in the course of work.
- Two years’ experience in behavioral health or health‑care settings.
- Experience working with psychiatrically impaired populations.
Ability to communicate and engage effectively with individuals from diverse backgrounds, cultures, education levels and socioeconomic statuses.
Physical, Sensory & EnvironmentalThis position involves daily community client services, travel between sites, driving, navigating varied terrain, frequent vehicle entry/exit, light lifting (10–20 lbs), computer use, and work in environments that may be less than ideal.
Pay RangeHourly Non‑Exempt: $25.89 – $31.53. Pay increases with longevity at FBH.
