Program Manager – Medicaid Care Coordination, Housing & Community Services

Washington State (Hybrid Remote with Statewide Travel)

About the Role

Mysti's Adult & Family Services – Washington is seeking an experienced Program Manager to oversee service delivery, staff performance, client outcomes, compliance, and community partnerships across Washington State.

This leadership position is ideal for someone who thrives in fast-paced environments, enjoys solving complex problems, and is passionate about helping individuals and families access housing, healthcare, Medicaid-funded services, employment supports, and other critical community resources.

The Program Manager will oversee intake operations, case management, care coordination, housing navigation, and program quality while supporting staff development, ensuring regulatory compliance, and strengthening partnerships with state agencies, Managed Care Organizations (MCOs), healthcare providers, housing organizations, and community partners.

This is a hands-on leadership position for someone who enjoys building programs, improving systems, coaching staff, and driving measurable outcomes.

What You'll Do
  • Supervise and support Intake Specialists, Case Managers, Housing Navigators, Referral Coordinators, Employment Specialists, and other program staff.
  • Monitor client engagement, service delivery, productivity, and successful client outcomes.
  • Oversee Medicaid-funded care coordination, housing navigation, employment supports, Community Choice Guide (CCG), Foundational Community Supports (FCS), and other community-based programs.
  • Ensure documentation meets Medicaid, Washington State, and organizational quality standards.
  • Review assessments, service plans, progress notes, referrals, authorizations, and case documentation for accuracy, timeliness, and compliance.
  • Support staff with high-acuity and complex client situations.
  • Advocate for clients with Managed Care Organizations, hospitals, behavioral health providers, housing providers, landlords, legal aid organizations, and community agencies.
  • Build and maintain strong relationships with healthcare systems, Accountable Communities of Health (ACHs), community organizations, and referral partners throughout Washington.
  • Organize and attend outreach events, resource fairs, coalition meetings, trainings, and community conferences.
  • Monitor program performance, identify operational challenges, and implement practical solutions.
  • Develop workflows, improve operational processes, strengthen accountability, and promote continuous quality improvement.
  • Collaborate with executive leadership to expand programs, improve service delivery, and support organizational growth.
  • Travel throughout Washington State to support staff, partners, community engagement, and program operations.
