Valid driver's license and reliable transportation is required for community-based work.
Effectively communicate with all stake holders: clients, clinicians, support team, insurance providers, DSHS, DVR, DDA, etc.
Conduct rapid job search activities beginning within 30 days of enrollment
Complete person-centered vocational assessments focusing on strengths, interests, and preferences
Develop individualized employment plans collaboratively with participants
Provide benefits counseling and work incentive planning
Assist with job applications, interview preparation, and workplace readiness
Conduct job carving and customized employment development as needed
Provide on-the-job coaching and workplace support
Negotiate reasonable accommodations with employers
Deliver follow-along supports for as long as needed
Facilitate career advancement and job transitions
Provide crisis intervention and problem-solving support
Assist with workplace relationship building and social skills
Develop and maintain relationships with local employers through face-to-face visits
Educate employers about benefits of hiring individuals with disabilities
Identify job opportunities that match participant preferences
Negotiate job accommodations and workplace modifications
Provide ongoing consultation to employers and supervisors
Assist with conflict resolution and workplace issues
Maintain comprehensive case files and progress notes using required documentation standards
Submit timely and accurate billing using appropriate procedure codes
Track employment outcomes and participant progress
Complete required reporting and data collection
Participate in quality assurance and program evaluation activities
Other duties as assigned
Bachelor’s degree in Social Services, or any related field
Relevant professional certifications are desirable, such as Certified Employment Support Professional, Employment Support Professional Certification, Certified Rehabilitation Counselor, or Commission of Rehabilitation Counselor Certification, or Certified Peer CounselorA minimum of 2 years’ relevant professional experience
Strong skills with technology (ex. Microsoft Windows and Office Suite)
Experience with creating and maintaining case notes
Demonstrated communication and networking skills
Ability to maintain confidentiality
Valid WA State Driver License and reliable transportation
