Hope Human Services is a premier provider of Supported Living services for developmentally & intellectually disabled adults in the State of Washington. The Case Manager participates as a member of the management team in maintaining quality and continuity of care. The person inthis position will, along with other members of the management team, provide leadership and guidance; and provide oversight to direct support professionals.

Responsible for staff development; parent and family education; learning individual scare prior to admittance and educating others on care; assisting with housing for individuals and admits. Works collaboratively with all other disciplines toensure all needs are met for the individuals. Responsible for hiring all direct support professionals, ensuring they are oriented and prepared to fulfill theduties and responsibilities of a direct care position. Supervises all direct support professionals, including progressive discipline, performance evaluation, competency, and compliance with policies and procedures.

Ensures compliance with all state regulations, including but not limited to licensing requirements, for the provision of care. Schedules staff for program and responsible for education and orientation on the individuals plans, and formsfor documentation. Participates in rotating, on call responsibilities providing support 24 hours per day, seven days per week.

Requirements & Essential job functions
  • Be atleast 21 years of age or older
  • Teaches and mentors staff to provide high level care in a safe environment
  • Demonstrates direct care skills, constant professionalism, critical thinking, and objectivity
  • Abilityto access the residences of the people we support, specifically those inthe caseload for the assigned program
  • Minimum high school diploma or GED;
    Associates or Bachelor’s degree in Human Services or related field preferred
  • One ormore years of supervisory experience and case management in Supported Living
  • Skills necessary to provide and teach individual care
  • Experience providing support for intellectually and developmentally disabled adults
  • Musthave a currently valid drivers license or valid drivers license as anactive duty armed forces personnel or dependent of an active duty armed forces personnel and the ability to be insured to drive agency vehicles
  • Proofof vehicle insurance
  • Provides driving abstract on annual basis or as requested
  • Provides valid CPR and First Aid or obtain it within 60 days of employment
  • Provides Supported Living letter of exemption or 75-hour training certificate orobtain it within 120 days of employment
  • Providesa peer coaching certificate or obtain it within 180 days of employment
  • Ableto pass a criminal background check with the State of Washington and maintain clearance during tenure of employment
  • Abilityto use email, word processing and spreadsheet software. Microsoft Office experience preferred.
  • Abilityto use Therap or similar program for online documentation of care of individuals supported
