In Home Healthcare LPN - Low Acuity Weekdays Shifts

Licensed Practical / Vocational Nurse – LPN/LVN

• Schedule: Mon‑Fri, 8 am‑4 pm
• Location: Tahlequah, OK
• Age Range: Teenager
• Acuity: Feeding tube & medication administration (training available)

• One‑to‑one patient care in a home environment
• Execute physician‑prescribed plan of care and document care in the system of record
• Administer prescribed medication, treatments, and therapies
• Perform patient assessments and coordinate care
• Provide health promotion, teaching, and training to family members

• Active, unencumbered LPN/LVN license in the state of practice
• Compact license transferred to state of residence within 90 days
• Current CPR certification with hands‑on component (training available after hire)
• TB skin test within last 12 months
• Six months prior hands‑on nursing experience preferred (not required)
• Reliable transportation
