We are seeking a detail-orientated and compassionate
* Home Health Admission Nurse (RN)
* to complete comprehensive start-of-care assessments for patients entering home health services. In this role, you will evaluate clinical needs, establish individualized care plans, and ensure a smooth, supportive transition into home-based care. The ideal candidate is confident in assessment skills, highly organized, and able to communicate clearly with patients, families, physicians, and the interdisciplinary team.
* Key Responsibilities
** Complete timely and accurate
* Start of Care (SOC)
* and admission assessments
* Develop individualized
* plans of care
* based on patients needs and clinical findings
* Educate patients and families on home health services, safety, and care expectations
* Coordinate with physicians and the care team to verify orders and service eligibility
* Document all assessments and clinical information in the EMR according to regulatory standards
* Identify patient risks, needs, and required interventions to support safe care at home
* Ensure a seamless handoff to ongoing case managers and field clinicians
* Qualifications
* * Registered Nurse (RN) with current licensure and experience in home health admission processes
* Strong clinical assessment, care coordination, and documentation skills (EMR/EHR) for care plans and nursing notes
* Knowledge of healthcare policies, patient care regulations, and collaboration with multidisciplinary teams
* Inpatient/outpatient/post-acute experience with focus on home health assessment and care transitions
* Ability to manage patient records, clinical documentation, and initial medical evaluations in a home setting
* Excellent patient interaction, critical thinking, and care management capabilities

Pay: $74,000.00 - $81,000.00 per year Benefits:
