Incident Management Process Consultant -

This is a highly responsible position serving the Agency for Persons with Disabilities (APD) Central Office whose role includes but is not limited to providing statewide leadership, coordination, technical assistance, oversight, and quality assurance related to Incident Management, zero tolerance initiatives, client health and safety, and other quality program functions within the APD service delivery system.

The Work You Will Do:

Serve as State Office lead for Incident Management and Zero Tolerance initiatives by providing statewide coordination, direction, and technical assistance to Regional Offices and providers regarding incident reporting, abuse, neglect, exploitation prevention, and client health and safety.

Manage and oversee critical incident and Zero Tolerance processes, including monitoring regional and provider reporting, follow-up activities, ensuring timely incident resolution and closure, providing technical guidance, and promoting compliance with federal and state requirements, Agency policies, and operational standards.

Collect, analyze, and interpret incident, investigative, and performance data from multiple sources, including critical incident reports and Department of Children and Families investigations, to identify trends, systemic risks, performance gaps, emerging vulnerabilities, and opportunities for quality improvement.

Conduct root cause analyses and develop recommendations to strengthen provider performance, client protections, and service quality.

Prepare and present routine and ad hoc reports, compliance reviews, and data analyses to Agency leadership to support informed decision-making, risk mitigation, policy development, and continuous quality improvement initiatives.

Lead or support the development, implementation, and evaluation of Zero Tolerance, abuse prevention, human trafficking awareness, and other health and safety initiatives, including the creation of training materials, educational resources, and statewide outreach efforts for APD staff, providers, clients, families, and external stakeholders.

Develop, revise, and implement policies, procedures, interagency agreements, and operational guidance to support compliance with regulatory requirements, strengthen cross-agency collaboration, improve information sharing, and enhance protections for APD clients.

Conduct quality assurance reviews, technical assistance activities, and monitoring of Regional Office operations to ensure compliance with applicable laws, rules, policies, and performance expectations, and provide recommendations for corrective actions and process improvements.

Research and evaluate industry standards, regulatory requirements, and best practices related to incident management, abuse prevention, neglect, exploitation, human trafficking, and client safety to support ongoing program enhancement and operational excellence.

Collaborate with Regional Offices, State Office leadership, providers, and partner agencies to implement strategic initiatives, rule making activities, compliance efforts, special projects, and other assignments that advance the Agency's mission of protecting the health, safety, welfare, and rights of the individuals served by APD.

Performs other duties as assigned.

Minimum Qualifications:

High school diploma or its equivalent Knowledge, Skills, And Abilities Knowledge of:
Fundamental operation of Microsoft Office platforms, skills in administrative writing and reporting, skills in process management and improvement. Knowledge In:
Process management. Data collection methods. Abuse, Neglect, and Exploitation prevention. Ability to: perform data analytics. Problem solve. Perform in a fast-paced, high volume work environment.

Preferred Qualifications:

Bachelor's degree preferred
